Middle School Registration

Welcome to Griswold Middle School! 

The following is a list of documents required for registration:

• 2 Forms of Proof of Residency - must have a current date
  • Mortgage Statement or Lease/Rental Agreement
  • Utility Bill or Homeowner's Insurance
• Copy of Student's Birth Certificate
• Copy of Parent/Guardian's ID or Driver's License
• Copy of Student's most recent report card from previous school
• Student’s Physical (within 1 year) & Immunization Record
• Griswold Middle School Registration Packet (all individual forms below)

Please complete and return the following forms to register:

• GPS Registration Form
• Release of Records Form
• Acceptable Use Policy (Tech)
• Health Office New Student Checklist
• Health Questionnaire
• Student’s Physical (within 1 year) & Immunization Record
• Tuberculosis Risk Form
• Postural Screening Form (only for female students in grade 5 and 7)
• Home Language Survey (only if student has not attended school in CT)

 

The following annual forms are also required to register (will be updated before each new school year):

• Annual Notice/Handbook Sign Off 2025/2026 (review the Student/Parent Handbook 2025/2026 here)
• Bus Transportation Form
• 1:1 Chromebook Agreement 2025-2026 (Tech)
• Certification of Immunizations (if applicable)
• Photo/Video Release
• Classroom Movie Permission Slip

* If information is missing on the above forms, additional forms will be required. 

If you qualify for Free or Reduced Lunch or CT HUSKY Health Insurance, please ask for more information and the required forms.

All documents and forms should be returned to the GPS Registration Office located in the Griswold Public Schools Central Office.

Please use the entrance on the north (playground) side of the building, accessible from the one-way loop on Slater Avenue in front of the school’s original facade.
